• The Tarrant location is currently in the credentialing phase and nearing the contracting stage. However, we are encountering the same issue as before—using the same address, same services, and the same providers as another application, which is not permitted.

Cook Children’s:

• We need to pause or remove the OBM application using the Tarrant location. According to the representative handling the application, Bowman is nearing the end of the contracting process. However, the rep, Rose, reached out via email to confirm OBM's address. It was noted that OBM was attempting to add a new practice location using the same providers for the same services with the same address, which is not permitted. I spoke with Evie, and we both agreed to pause the addition of the OBM location until a new, compliant location can be identified.
